According to Salem High School’s Athletic Office, free physicals will be offered on May 11 and 18.
Free physicals will be administered by local doctors on Wednesday, May 11 for boys and Wednesday, May 18 for girls in the Randy Johnson Fieldhouse at 5:30, respectively.
Students planning to participate in a sport, whether it be fall, winter or spring, during the 2016-2017 school year must have a physical form on file in the athletic office in order to participate.
Please stop by the athletic office to pick up a physical form before the day of physicals to complete page one and acquire parent/guardian signatures.
